I understand that I will receive traditional medspa or cosmetic treatments from Valeriya Life, which operates under a medical license. I understand that depending on the treatment I select, I will be required to sign an informed consent specific to that treatment. I am fully aware that my condition is solely of a cosmetic nature and that the decision to proceed is based on my expressed desire to do so.
Payment Policy
I understand that my treatments at Valeriya Life require payment and the prices and fee structure for treatment have been explained to me. I understand that the services often require more than one session as well as recommended lifestyle modifications for the best outcome. I have the option of purchasing a series/package of treatment sessions at the quoted package price. There is no guarantee of refunds on treatments paid in advance. I agree to pay for the treatment according to the payment plan discussed.
Late Cancellation, Rescheduling and No-Show Policy
I am aware that Valeriya Life allows rescheduling the appointment up to 24 hours in advance and up to 3 times. I am aware that Valeriya Life requires to text or call to cancel or reschedule appointments. If I cancel or reschedule within 24 hours of the start of the scheduled appointment time will be subject to a charge of $25. Failure to notify a member of Valeriya Life for missed appointments (no call/no show) before the start of the scheduled appointment time will be charged $50.
No Refunds for Services and Treatments Policy
I understand that there are no refunds for services or treatments. If I decide to cancel, I will receive a gift card with the remaining balance. I also understand that no transfers of benefits are available. If I experience a change in my medical history or use of prescription medications, that may prevent me from receiving a treatment or service that was already paid for, then I must obtain a letter from my doctor detailing the change in medical condition or use of medications. This updated information will be reviewed for deciding if a change in treatment or refund of unused services or treatments is warranted.
Return Policy for Products
I am aware that if I am not completely satisfied with a product for any reason, I should contact Valeriya Life within 30 days of the purchase date. Valeriya Life will go over my concerns and chat about the best way to get the most out of my purchase. If Valeriya Life can’t make this product work for me, then they will arrange for a return or exchange of the item. All returns/exchanges need to be made within 30 days of purchase. Valeriya Life accepts one individual item to be returned or exchanged in a 365-day period. After this return or exchange, I may make another once 365 days have passed from the date each individual return or exchange was processed. All returned merchandise must be in its original packaging. All parts, pieces, cords, chargers, printed materials, promotional items, gifts with purchase and any original product packaging must be returned. (You may keep free gifts and gifts with purchase, but we may charge your account for the full value of the gift.) Returned products may not be altered and must contain over 90% of the product. Valeriya Life reserves the right to deny the return or issue a store credit if the item is not returned complete with original packaging.
Interest Free Financing
Clients who spend $500 or more on the date of service have the option of using our “in-house interest free financing” plan. For interest free financing, you will pay 50% of the total amount due on the day of service, and another 25% of the amount due at 30 and 60 days after the date of service. There is a finance processing charge of $15 to each finance payment. You will need to fully complete a legally binding Financial Agreement and provide a credit or debit card account to be used for financed payments. We will keep a copy of your Driver’s license on file for our reference. If I am late in making payment, I will receive a Registered Letter from us after 30 and 60 day payment are overdue. Accounts that are 30 days overdue will be charged an additional 2% per month interest on the balance that is due. Accounts that are 60 or more days overdue will be taken to Small Court in Douglas County, NE.

I understand that all cosmetic treatments are provided exclusively by Valeriya Life for aesthetic purpose. I will not hold Valeriya Life, it’s owners or its employees responsible for the results I experience. I realize that results may vary. I further understand that Valeriya Life cannot prescribe an exact number of treatments to satisfy everyone’s opinion and that the number of treatments I complete will be at my own discretion. I understand that even with the best technology and highest trained technicians, some people will not have a desired response/outcome to treatment.

I understand that for the treatment plan staff will take pictures of my face and/or body before, during, and after treatment for tracking my progress.
Privacy
I have received a copy of the Valeriya Life Notice of Privacy Practice. I have read and fully understand all the terms of this Acknowledgment of Practice Policies, all my questions have been answered to my satisfaction and I agree to the terms of this consent:
